Posterior-access neck lift: when the scar stays hidden

In consultation, the question is rarely “I want a neck lift.” It tends to be more specific: the neck bands are showing, the angle between the jaw and neck has been lost, and the person wants to know whether a scar in front of the ear is necessary. The posterior-access neck lift is an alternative in selected cases. It does not replace a face and neck lift when the cheek has also dropped.

In facial surgery, Mallorca Medical Group assesses the neck as a single anatomical unit: skin, fat, platysma muscle, and jaw position. Dr. García Ceballos, MD, FCCP and MRCS I England, a specialist in plastic, reconstructive, and aesthetic surgery, has 25 years of clinical practice. The indication is decided in consultation, not from a photo.

What anatomical problem it addresses

Over the years, the platysma can separate along the midline and form vertical bands. The skin of the neck loses elasticity. Sometimes there is excess fat under the chin that does not respond to body weight. The visible result is a less defined cervical angle, not a generic “aged neck.”

The posterior access aims to treat that plane in patients whose main problem is in the neck while the jawline remains reasonably acceptable. The incision is designed to stay hidden behind the hairline, instead of a conventional anterolateral approach. That does not mean there is no scar. It means that, in the right candidate, it stays out of frontal view.

When it makes sense and when it doesn’t

It makes sense when:

Platysmal bands are the main reason for consultation.

The skin still has reasonable elasticity, or the excess skin is limited and localized.

The cheek and nasolabial fold do not call for a facelift at the same time.

The patient accepts that aging continues and that the neck is not “fixed” in place.

It is not the right option if the cheek has clearly dropped, if there is significant excess skin that can only be redistributed with a more extensive approach, or if the person expects to completely erase fine skin wrinkles. In those cases, a combined face and neck lift may be more coherent than operating on the neck alone and leaving the cheek sagging.

Nor does it replace chin liposuction when the problem is mainly fat, with firm skin and no muscle banding. Mixing indications is a frequent cause of disappointment.

What’s Changing and What Isn’t

What can improve, if the indication is correct, is the angle between the jaw and neck, the tension of the bands, and the continuity of the jawline seen from the front and in profile. The change is progressive. The swelling of the first few weeks is not the result.

What does not change: skin quality, blemishes, chin fat that depends on body weight, or the aging of the rest of the face. The Spanish Society of Plastic, Reconstructive and Aesthetic Surgery (SECPRE) notes that aesthetic surgery is not a treatment for the skin and that every indication must be individualized. A hidden scar is not a nonexistent scar: its location, maturation, and care are explained.

Recovery, with no magic timelines

In the first few days there is swelling, a feeling of tightness, and often a compression garment or chin strap depending on the plane operated on. Usual social activity resumes gradually, not the next day. Exercise and sun exposure are guided in consultation. The result is judged over months, as the tissue settles, not in the first look in the mirror.

Risks exist: hematoma, temporary changes in sensitivity, asymmetry, healing problems, and, infrequently, injury to a nerve branch. No neck procedure is “risk-free” or comes with a guaranteed result. The clinical goal is the best possible outcome for that patient, not a transformation.

How Decisions Are Made in a Consultation

The assessment includes examination while standing and seated, skin quality, platysma position, submental fat, and harmony with the jaw. Medical history, medication, and expectations are also reviewed. If the posterior access is not a good fit, the patient is told so. Sound clinical judgment includes pointing out what should not be operated on.

Consultations at Mallorca Medical Group are not free of charge. They can be booked at mallorcamedicalgroup.com. Every case is individual, and this information does not replace that assessment.

Double Chin: When Liposuction Is Enough and When a Neck Lift Is Needed

A double chin is not a diagnosis. It is a contour issue: the angle between the jaw and the neck becomes blurred, and the profile is no longer clearly defined. In clinical practice, this complaint encompasses various causes. Sometimes submental fat predominates, with skin that is still elastic. Other times, the skin has lost its support, the platysma has separated into bands, and the cervicomentonian angle can no longer be restored by removing fat. Confusing these two cases is the most common mistake when seeking to “remove the double chin” as if it were a single procedure.

At Mallorca Medical Group, Dr. José Ignacio García Ceballos, MD, FCCP, and MRCS (England), evaluates this contour before recommending a procedure. He has 25 years of clinical experience in plastic, reconstructive, and cosmetic surgery. The purpose of this article is to explain the difference between cervical liposuction and a neck lift, not to promise a specific result.

What’s Really Visible Under the Double Chin

Three planes intersect at the neck and do not respond the same way to the same surgery.

Submental fat. This can be constitutional, not just the result of weight gain. If the skin returns to its normal position when pinched and there are no muscle bands, cervical liposuction may be sufficient.

Skin laxity. Excess skin remains even after fat is removed. In this case, suctioning without skin repositioning leaves a hollow and, at times, irregular contour.

Platisma. The vertical bands on the neck are muscle, not fat. They do not disappear with a cannula. They require surgical intervention on the muscle, as part of a neck lift or a cervicofacial lift.

Age is a factor, but it is not the deciding factor. There are young patients with loose skin following significant weight loss, and older patients with localized fat deposits and good skin elasticity. The in-office examination—jaw position, subcutaneous fat thickness, skin quality, and platysma tone—carries more weight than a profile photo sent via text message.

When Neck Liposuction Is Appropriate

Double chin liposuction is indicated when the main problem is excess fat and the skin can be retracted. The typical candidate has a blunt cervicomentonian angle, little or no platysmal band, and skin elasticity that is confirmed by physical examination—not merely assumed based on age.

The procedure removes fat from the submental compartment using fine cannulas through incisions a few millimeters long under the chin and, if necessary, behind the earlobe. It does not tighten the skin or suture the platysma. For this reason, it is not recommended as a standalone solution if there is already skin sagging or pronounced bands: removing fat in this context can accentuate the sagging.

Nor does it replace weight loss. As with other areas, liposuction reshapes localized fat deposits; it is not a treatment for obesity. If your weight is not stable, your body contour may change again.

The typical recovery process includes wearing a cervical compression garment for the first few days, swelling that gradually subsides over two to four weeks, and a more clearly defined contour by the second or third month. Bruising, irregularities, and insufficient skin retraction are real risks. They are not theoretical exceptions. They are discussed before surgery.

When Your Neck Needs a Facelift, Not Just a Cannula

A neck lift is indicated when there is excess skin, the platysma has separated, or the neck contour cannot be restored by removing fat. The Mallorca Medical Group’s page on facelifts and neck lifts describes two approaches that Dr. García Ceballos uses depending on the case: the cervicofacial lift, which addresses both the SMAS layer and the neck simultaneously, and the posterior-approach neck lift, with the scar hidden behind the hairline, designed for necks with marked laxity and little need for intervention in the middle third of the face.

The posterior approach is not “the best technique.” It is an alternative in select cases. If facial and cervical sagging occur together, a procedure focused solely on the neck leaves a discrepancy between the jaw and the cheek. If the problem is almost exclusively cervical, performing a full facelift may be more surgery than the case warrants. That is a discussion for the consultation, not a point for a brochure.

The Spanish Society of Plastic, Reconstructive, and Aesthetic Surgery (SECPRE) reminds the public that these procedures must be recommended by a qualified plastic surgeon following an examination, and that the outcome depends on the patient’s initial anatomy. There is no such thing as a “standard” neck.

What the doctor must rule out before performing surgery

Not every double chin requires surgery, and not every surgical procedure for a double chin is cosmetic.

A pronounced mandibular retrusion can mimic a double chin. In that case, removing fat does not create an angle that the jaw cannot support. Sometimes the discussion turns toward a mentoplasty; other times, toward not performing surgery.

A prominent submandibular gland, very thin skin, or previous scarring can change the treatment plan.

Energy-based devices and threads can improve very early-stage laxity. They do not correct a descended platysma or significant skin excess. Presenting them as the equivalent of a facelift is an oversimplification.

The article on when a facelift is indicated applies that same criterion to the face: the indication is based on anatomy, not on a specific age cutoff. The same applies to the neck.

What to Expect—and What Not to Expect

Properly indicated neck liposuction reduces volume under the chin. It does not eliminate muscle bands or remove excess skin. A neck lift repositions the skin and, when indicated, the platysma muscle. It does not stop the aging process or guarantee an angle identical to that of a reference photograph.

The scars from a neck lift are designed to lie within skin folds or behind the hairline. “Hidden” does not mean invisible under all lighting conditions or with all hairstyles during the first few weeks. Swelling in the neck is noticeable in the first few days and does not reflect the final result.

Each case is unique. The same profile photo could correspond to liposuction, a posterior-approach neck lift, a cervicofacial lift, or the decision not to undergo surgery. That decision is made during a consultation, based on a physical examination, not based on a message.

Anyone who would like to learn more about how this assessment is conducted can review Dr. García Ceballos’s professional background: training in the British NHS (MRCS I England), a residency at the Université Libre de Bruxelles, and a fellowship at the Collegium Chirurgicum Plasticum (FCCP). Appointments can be booked at mallorcamedicalgroup.com. This is not a free consultation; it is a personalized evaluation.

Facial Rejuvenation in Mallorca: Surgery vs. No-Scalpel Treatments – Complete Guide by Dr. Garcia Ceballos FCCP

By Dr. José Ignacio García Ceballos FCCP – Plastic, Reconstructive and Aesthetic Surgeon at Mallorca Medical Group

Published on April 20, 2026

The passage of time leaves its mark on the face in many different ways: loss of volume, appearance of wrinkles, skin flaccidity, changes in skin texture and alterations in facial contours. Faced with this reality, patients today find themselves with an unprecedented range of options: from aesthetic medicine treatments to highly complex surgical interventions. However, the abundance of options often generates confusion. When is a non-surgical treatment sufficient? When is surgery the only real and lasting solution? How do the two approaches complement each other? In this comprehensive guide, Dr. Garcia Ceballos FCCP, plastic surgeon in Mallorca, answers all these questions with clinical judgment and real-world experience.

“The decision between surgery and nonsurgical treatments should not be based on trends or fear, but rather on an objective analysis of the changes the patient is experiencing, their actual goals, and the scientific evidence regarding the long-term results of each option.”

— Dr. José Ignacio García Ceballos FCCP, Plastic Surgeon at Mallorca Medical Group

1. Facial Aging: A Three-Dimensional Process

To understand which treatment is appropriate in each case, it is essential to understand how the face ages. Facial aging is not simply the appearance of superficial wrinkles: it is a three-dimensional process that affects all layers of the face simultaneously.

At the cutaneous level, there is a progressive reduction of collagen and elastin, leading to loss of firmness and the appearance of expression lines. At the subcutaneous level, the facial fat compartments – which in youth are well defined and occupy elevated positions – are displaced downward and unevenly redistributed, creating furrows and loss of volume. In the deeper structures, the facial bony skeleton undergoes progressive resorption, especially in the ocular orbit, cheekbone and maxilla, reducing the structural support of the overlying soft tissues.

This three-dimensional understanding of aging is the basis on which Dr. García Ceballos designs individualized treatment plans at Mallorca Medical Group: it is not about erasing wrinkles, but about restoring the architecture of the face.

2. Non-Surgical Rejuvenation: Indications, Advantages and Limitations

Facial aesthetic medicine treatments have undergone an extraordinary advance in the last decade. Today we have tools that, correctly used, allow us to obtain very satisfactory results in certain situations. However, each treatment has a precise clinical indication range.

Botulinum toxin (Botox): Effective for dynamic wrinkles in the upper third of the face (frown, forehead, crow’s feet). Its action is temporary (4-6 months) and does not resolve eyelid ptosis, excess skin or structural flaccidity. Competing clinics in Mallorca such as eonclinicpalma.com or medisans.com offer these treatments, but the difference in Mallorca Medical Group lies in the previous evaluation by a plastic surgeon who determines if the patient will really benefit from the procedure or requires a deeper approach.

Hyaluronic acid and fillers: Indicated to restore lost volume in cheekbones, nasolabial folds and lips, and to treat dark circles under the eyes (tear trough). They are especially useful in young patients with incipient volume loss. In patients with advanced flaccidity, fillers may worsen the appearance if not accompanied by structural treatment.

Collagen biostimulators (Sculptra, Radiesse, Profhilo): They stimulate the production of the patient’s own collagen, with more natural and long-lasting results (12-24 months) than conventional fillers. They are an excellent option as a post-surgical complement or in patients who wish to improve skin quality without surgery.

Energy technologies: Radiofrequency, focused ultrasound (HIFU) and fractional laser are indicated for mild to moderate flaccidity and improvement of skin texture. Their results are modest compared to surgery and do not correct excess skin.

The fundamental limitation of all non-surgical treatments is that they cannot address excess skin or reposition ptotic tissues. When ptosis is moderate or severe, attempting to treat it with aesthetic medicine alone leads to unsatisfactory results and, in many cases, an artificial appearance.

3. Surgical Rejuvenation: When is it the Definitive Solution?

Facial rejuvenation surgery remains the only approach capable of providing real and lasting correction of excess skin, soft tissue ptosis and structural flaccidity of the middle and lower third of the face.

Facelift (rhytidoplasty): Paradigmatic intervention of surgical rejuvenation. At Mallorca Medical Group, Dr. García Ceballos performs the deep SMAS and Deep Plane technique, which acts on the muscular and fascial layers of the face to obtain natural and long-lasting results (10-15 years), far superior to those of superficial techniques that only tighten the skin. Unlike what some competitors offer, the Deep Plane approach avoids the “stretched” appearance characteristic of superficial lifts.

Blepharoplasty: Eyelid surgery corrects upper eyelid skin excess, palpebral fat bags and lower eyelid laxity. No non-surgical treatment can remove the actual excess eyelid skin; attempting to do so with radiofrequency or plasma technologies achieves only very limited and temporary results.

Neck lift and neck liposuction: The neck is often the first area where aging becomes visible. The combination of neck lift and jowl liposuction provides results that no minimally invasive treatment can match.

The experience of Dr. García Ceballos, with training at the Plastic Surgery Unit of the UZ VUB in Brussels and FCCP degree, guarantees a surgical planning based on the most updated anatomical principles.

4. The Combined Strategy: The Modern Approach to Facial Rejuvenation

The “surgery vs. non-surgery” dichotomy is largely artificial. The contemporary approach to high quality facial rejuvenation, as practiced by Dr. Garcia Ceballos in Mallorca, is combined: surgery is used for what only surgery can resolve (excess skin, structural ptosis) and is complemented by aesthetic medicine for what aesthetic medicine resolves excellently (volume loss, skin quality, dynamic wrinkles).

This approach is far superior to that offered by centers specialized only in aesthetic medicine (such as institutorubi.com or mesomedic.es) which, lacking surgical capacity, are conditioned to treat with fillers and toxin situations that would really require surgery, with the risk of overloading the face with product and obtaining artificial results.

It is also superior to that of centers that only offer surgery and do not have a quality aesthetic medicine unit for the maintenance of postoperative results.

5. Choosing the Right Treatment Plan: The Evaluation at Mallorca Medical Group

Choosing the optimal treatment plan requires a detailed clinical evaluation that includes:

Three-dimensional facial analysis: Evaluation of the three thirds of the face, identification of the main cause of apparent aging (loss of volume, ptosis, loss of skin quality or combination).

Complete medical history: medical history, previous aesthetic treatments (accumulated fillers, toxin), medication, habits (smoking, sun).

Standardized photographic documentation: Photographs in multiple projections with controlled lighting for planning and monitoring.

Patient expectations: Honest discussion about what each treatment can achieve and in what time frame, avoiding both therapeutic oversupply and undersupply.

At Mallorca Medical Group, unlike what happens in centers that prioritize the volume of procedures, each assessment is performed personally by Dr. García Ceballos FCCP, guaranteeing rigorous clinical criteria and an objective recommendation.

6. Expected Results and Longevity of Each Approach

One of the most frequently asked questions from patients is how long the results of each option last. The honest answer is that it depends on the degree of aging treated and the type of intervention performed:

Botulinum toxin: 4-6 months

Hyaluronic acid fillers: 9-18 months depending on the area.

Collagen biostimulators: 12-24 months

Deep Plane Facelift: 10-15 years

Blepharoplasty: Permanent upper eyelid; lower eyelid may require revision at 10-15 years of age

This difference in longevity is a fundamental factor in the real cost-benefit assessment of each option. A patient who undergoes repeated fillings every year for ten years can accumulate an economic cost and a risk of complications (granulomas, product migration) much higher than those of a well-performed surgical intervention.

Frequently Asked Questions about Facial Rejuvenation

<details>

At what age is it appropriate to consider a facelift?

There is no specific age: the indication depends on the changes present, not on the number of years. In general, the patients who benefit most from a facelift are between 45 and 65 years old, although there are cases in which it is indicated earlier (genetic factors, massive weight loss) or later. The determining factor is the clinical examination, not the chronological age.

</details>
<details>

Can I combine surgery and aesthetic medicine on the same day?

In many cases it is possible and convenient. For example, a blepharoplasty can be combined with botulinum toxin in the same session. A facelift can be complemented with lipofilling (fat grafting) during the same surgery to restore volume while repositioning the tissues. Dr. Garcia Ceballos designs the most efficient combined plan for each patient.

</details>
<details>

What are the risks of facial aesthetic medicine treatments?

Although aesthetic medicine treatments are perceived as safe, they are not free of risks: vascular emboli due to inadvertent injection of fillers in vessels, granulomas, asymmetries, transient eyelid ptosis due to poorly placed botulinum toxin. Therefore, it is essential that they are performed by a physician with specific training and deep anatomical knowledge of the facial territory. At Mallorca Medical Group, all aesthetic medicine procedures are performed or supervised by Dr. García Ceballos FCCP.
