Gynecomastia that persists after weight loss: what remains and what exercise cannot resolve

Losing weight reduces fat, but not always breast volume. In consultation, Dr. García Ceballos, MD, FCCP and MRCS I England, sees men who have achieved sustained weight loss yet retain breast definition. This residue cannot be corrected with more exercise. The useful question is not whether the gynecomastia “returns,” but what tissue remains and whether surgery is indicated.

Gynecomastia is the increase in breast volume in males. It can be primarily fat, primarily gland, or a mixture. Fat responds, in part, to weight. The gland is not “burned off” in the gym. This is why a well-managed weight loss can flatten the chest in one case and leave it almost unchanged in another.

What usually remains after weight loss

Following stable weight loss, the examination distinguishes three situations:

Fat predominance. Volume recedes significantly. If the skin is elastic, surgery is sometimes unnecessary.

Glandular component. A firm disc remains under the areola. Exercise does not eliminate it. Liposuction alone does not either, because this tissue is dense.

Excess skin. Volume decreases and a fold or wide areola remains. Here the issue is no longer just the content, but the envelope.

The Spanish Society of Plastic, Reconstructive and Aesthetic Surgery (SECPRE) describes gynecomastia surgery as an option for healthy men with elastic skin when the excess gland or fat is not tolerated and is not corrected by lifestyle habits. It is not a shortcut for untreated overweight.

When it does not yet make sense to operate

Surgery is not indicated if weight is still dropping, if there is a drug or habit maintaining the stimulus (anabolic steroids, cannabis, excessive alcohol), or if a medical cause has not been assessed. In these cases, operating before stabilizing the condition increases the risk of the volume reappearing.

Nor is it indicated as a substitute for a weight loss plan. Chest liposuction does not treat obesity. If the body mass index remains high and the distribution is general, the first step is medical, not surgical. At Mallorca Medical Group, this distinction is made during the assessment, not in an advertisement.

What surgery can correct, and what it cannot

When weight has been stable for months and the examination confirms glandular tissue, localized fat, or excess skin, the technique is chosen based on that finding:

Glandular resection through an incision at the lower edge of the areola, for cases involving a firm disc.

Liposuction if fat predominates, sometimes as a complement to resection, not as a substitute.

Skin resection and, in advanced cases, repositioning of the areola-nipple complex. The scar is more visible. This must be stated beforehand.

Surgery can result in a flatter chest. It does not guarantee perfect symmetry, does not erase all scarring, and does not prevent a recurrence if hormonal stimuli, anabolics, or significant weight gain return. The result is assessed over months, not upon removing the bandage.

Typical recovery includes a compression garment for several weeks, initial limitation of pectoral exercise, and wound review. Hematoma, seroma, changes in sensitivity, and a wider-than-expected scar are real risks, not theoretical exceptions. Each case is decided in consultation.

How it is approached in consultation

During the first visit, medical history, medication, habits, and weight stability are reviewed. The chest is palpated to distinguish between fat, gland, and skin. If necessary, an endocrine assessment is requested before discussing the operating room. The plan, if any, comes from that examination, not from a photo.

Dr. García Ceballos is a specialist in plastic, reconstructive, and aesthetic surgery, with 25 years of clinical practice, a Fellow of the Collegium Chirurgicum Plasticum (FCCP), and MRCS I England. Procedures are performed at Hospital Quirónsalud Palmaplanas and Hospital Quirónsalud Son Verí. The consultation is not free: it is booked at mallorcamedicalgroup.com.

If the volume remaining after weight loss is bothersome and weight is already stable, the assessment serves to determine if gland, skin, or both remain, and if surgery offers something that exercise can no longer provide.
