Tissue does. A 38-year-old with malar descent may have a clearer indication than a 55-year-old with good elasticity and a volume problem only.<\/p>\n\n<p>In practice, surgery is considered when several of these findings coincide:<\/p>\n<ul>\n<li>Midface descent that volume replacement alone does not correct.<\/li>\n<li>A blurred jawline or early jowl, with or without platysma bands.<\/li>\n<li>Elasticity that still allows a conservative skin resection.<\/li>\n<li>A realistic aim: better support, not a different identity.<\/li>\n<li>Stable weight and no relevant medical contraindication.<\/li>\n<\/ul>\n\n<p>It is generally not indicated when the main concern is a fine expression line, a fold that responds to filler, or a wish to look like someone else. It starts in support. Before 45, three changes tend to predominate:<\/p>\n\n<p><strong>Descent of fat compartments.<\/strong> The cheek loses projection and the nasolabial fold deepens even when the skin is still elastic. Treating only the dermis leaves the underlying problem.<\/p>\n\n<p><strong>Laxity of the SMAS and platysma.<\/strong> That layer supports the midface and neck. If it gives way, pulling skin alone produces a tight, short-lived result.<\/p>\n\n<p><strong>Skin and habits.<\/strong> Sun damage, smoking, weight change and family history shift the timetable. Two people of the same age do not have the same tissue. That is why there is no universal minimum age.<\/p>\n\n<p>The <a href=\"https:\/\/secpre.org\/estiramiento-facial\" target=\"_blank\" rel=\"noopener\">SECPRE note on facelift<\/a> recalls that the operation acts on skin, fat and muscle, and that indication is individual. It does not fit every young face. If the neck or the cheek needs a wider vector, a short operation falls short.<\/p>\n\n<p>Deep plane is not chosen by age or by trend. It is chosen when descent sits in ligaments and compartments, not only in skin. Surgery is longer and early swelling can be greater. The aim is to reposition, not to tighten the dermis.<\/p>\n\n<p>The posterior-approach neck lift, an alternative described by Dr Garc\u00eda Ceballos for selected cases, hides the scar behind the hairline when the neck is the dominant problem. It does not replace a midface lift.<\/p>\n\n<blockquote>\n<p>In a younger patient, the question is not whether surgery is possible. It is whether surgery corrects the tissue that has given way, and whether a conservative resection is enough. It includes history, examination of the midface, jawline and neck, skin quality and medical photographs. It also reviews what bothers the patient, what will not be touched, and what can worsen if the wrong technique is chosen.<\/p>\n\n<p>When relevant, the conversation also covers:<\/p>\n<ul>\n<li>Scarring history, smoking and recent weight change.<\/li>\n<li>Previous fillers or threads, which can alter planes.<\/li>\n<li>Whether blepharoplasty is needed: that is another operation and another recovery.<\/li>\n<li>The setting of <a href=\"https:\/\/mallorcamedicalgroup.com\/en\/cirugia-facial-mallorca\/\">facial surgery in Mallorca<\/a>: hospital, anaesthesia and follow-up, not only the technical gesture.<\/li>\n<\/ul>\n\n<p>Every operation has risks: haematoma, temporary change in sensation, a visible scar or asymmetry. They are reduced by a correct indication, a hospital theatre and real follow-up. A deep plane may need longer.<\/li>\n<\/ul>\n\n<p>Scars sit at the hairline and around the ear, and on the neck only if needed. In younger skin, scar quality is often favourable, but it is not uniform.<\/p>\n\n<p>Duration is not indefinite: tissue keeps ageing. In many patients the change lasts for years, with an orientative range of 8 to 12 depending on technique, skin and habits. That is not a promise of outcome.<\/p>\n\n<p>Naturalness is the clinical aim: definition that others notice, not a different face. If the problem is loss of support, filler can camouflage it for a time and, in excess, widen the face. Threads are not equivalent to a facelift.
