It is not localised fat, and it is not corrected by diet or by liposuction. In consultation, Dr García Ceballos, MD, FCCP and MRCS I England, with 25 years of practice in plastic, reconstructive and aesthetic surgery, assesses whether that separation explains the abdominal bulge and whether an abdominoplasty is indicated, or whether rehabilitation should come first.

What it is, and what it is not

The rectus muscles meet at the linea alba. When that band stretches — most often after one or more pregnancies, and also after a large weight gain — a gap appears in the midline. The abdomen may bulge on sitting up, on coughing, or at the end of the day, even when the scales do not explain it.

A mild separation can improve with directed work of the transversus and the pelvic floor. A wide separation, with spare skin and a wall that does not contain the abdominal contents, does not close with exercise. It does not close by suctioning fat either: liposuction treats the fat layer, not the fascia.

Clinical examination measures the gap at rest and on effort, above and below the umbilicus. If there is doubt, an ultrasound of the abdominal wall helps to distinguish diastasis, umbilical hernia, or both. A hernia, if present, is planned in the same operation or referred according to its size. It is not a minor aesthetic detail.

When surgery is not the first step

There is no sense in operating on rectus diastasis if weight is not stable, if a pregnancy is planned in the short term, or if the main symptom is low-back pain without a clearly incompetent wall. In those cases, pelvic-floor and abdominal-wall physiotherapy is the reasonable path. Operating too early exposes the repair to recurrence when the wall stretches again.

Abdominoplasty is also not indicated if there is no spare skin and the separation is small. An isolated plication can be considered in very selected cases, but it is not the usual answer. The aim is not to “close a number of centimetres”: it is to recover a competent wall and a contour that matches that person’s tissues.

When abdominoplasty is indicated

The indication appears when three findings coincide: a separation that persists on effort, redundant skin below the umbilicus or across the abdomen, and a bulge that does not yield to rehabilitation. In a full abdominoplasty, excess skin is removed, the umbilicus is repositioned, and the rectus fascia is approximated with suture. Repair of the diastasis is part of that step, not a cosmetic add-on.

A mini-abdominoplasty only reaches the skin below the umbilicus. It does not correct a separation above the navel. Confusing the two techniques is a frequent cause of an incomplete result. If the separation extends above the umbilicus, a full approach is required.

The scar sits low, along the pubic line, and its length depends on how much skin must be removed. A short scar cannot be promised if the excess is wide. Dr García Ceballos explains this at the assessment, together with the position of the umbilicus and the quality of the skin.

What it does not solve

Surgery does not replace weight maintenance. Later weight gain stretches the skin again and can reopen the linea alba. It does not treat stretch marks selectively: only those within the resected skin disappear. Back pain may improve if it was due to an incompetent wall, but that is not a guarantee and not a reason on its own to operate.

Recovery is not that of a liposuction. There are drains or a dressing according to the case, a garment for several weeks, and a real limit on sitting up and on lifting. A return to desk work is often around two to three weeks; physical effort comes later. Each case is adjusted in follow-up.

How the decision is made in consultation

At Mallorca Medical Group the decision does not come from a photograph. It comes from examination, from obstetric or weight-loss history, from smoking, and from whether the family is complete. Smoking raises the risk of skin compromise and wound-healing problems; it is stopped before surgery.

Planning an abdominoplasty in Mallorca distinguishes the full procedure, the extended procedure to the flanks, and the addition of liposculpture only when there is localised fat as well as diastasis. Suctioning fat does not replace the plication. Combining both steps needs a clear criterion so as not to compromise skin blood supply.

The Spanish Society of Plastic, Reconstructive and Aesthetic Surgery notes, in its information for patients, that aesthetic surgery should be indicated individually and by an accredited specialist. That is also the position of this practice: there is no single technique and no identical result for every abdominal wall. A clinical review of the anatomy of diastasis, published in Plastic and Reconstructive Surgery, also underlines that wall protrusion matters as much as the width of the gap when a repair is being considered.

If the question is whether a bulging abdomen is fat, skin, or rectus diastasis, the answer is in the examination, not in an advertisement. The appointment with Dr García Ceballos is also for saying when not to operate.

Revisión médica y transparencia editorial

Revisión médica: Dr. Garcia Ceballos, 070707770, specialist in plastic, reconstructive and aesthetic surgery

Última revisión: 12/10/2026

Este contenido ha sido sometido a revisión humana y control editorial antes de su publicación.
