class=\"ez-toc-section-end\"><\/span><\/h1>\n\n<p><strong>Lower blepharoplasty<\/strong> is surgery of the lower eyelid. The usual indication is a fat bag that bulges beneath the eye, sometimes with excess skin. It is not a treatment for pigmented dark circles, not a facelift, and not a way to erase tiredness in a single session. In consultation, Dr Garc\u00eda Ceballos, MD, FCCP and MRCS I England, with 25 years of practice in plastic, reconstructive and aesthetic surgery, first distinguishes which component predominates: fat, skin, laxity of the lid margin, or a shadow in the tear trough. Technique follows that reading, not a reference photograph.<\/p>\n\n<p>In Spain, the related query \u201cbolsas en los ojos\u201d (bags under the eyes) holds a stable monthly volume in Keyword Planner, around 3,600 searches, with peaks of 4,400 between late 2025 and early 2026. Interest is not a surgical indication. Many bags are constitutional and stable. The result settles over months. Swelling in the first weeks is not the result. Neither is millimetre symmetry: the two eyelids were not identical before surgery.<\/p>\n\n<h2><span class=\"ez-toc-section\" id=\"What_it_does_not_correct\"><\/span>What it does not correct<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n<p>The limits belong next to the indication:<\/p>\n<ul>\n<li>A dark circle caused by skin pigment does not disappear when fat is removed.<\/li>\n<li>Drooping of the upper eyelid is a different problem. If that predominates, upper blepharoplasty is assessed, alone or combined. It is not the subject of this article.<\/li>\n<li>Dynamic crow\u2019s feet are not treated by resecting lower-lid skin.<\/li>\n<li>A hollow tear trough can worsen if too much fat is removed. Sometimes fat is preserved or repositioned, not emptied.<\/li>\n<li>Laxity of the lid margin (a lid that pulls away from the globe) requires assessment of additional support. Operating on the bag alone, without looking at that support, raises the risk of retraction or a round eye.<\/li>\n<li>Surgery does not replace sleep, allergies, or contact dermatitis. Those factors are reviewed before any indication.<\/li>\n<\/ul>\n\n<p>The <a href=\"https:\/\/mallorcamedicalgroup.com\/en\/blog\/plastic-surgery-en\/clasificacion-parpado-inferior\/\">classification of the lower eyelid<\/a> published on the clinic\u2019s blog separates dark circles, bags and excess skin so treatments are not mixed. That classification is a guide for conversation, not a closed protocol.<\/p>\n\n<h2><span class=\"ez-toc-section\" id=\"Two_approaches_not_two_%E2%80%9Cstyles%E2%80%9D\"><\/span>Two approaches, not two \u201cstyles\u201d<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n<p>There are two usual routes. Neither is better in the abstract.<\/p>\n\n<p><strong>Transconjunctival approach.<\/strong> The incision lies on the inner surface of the eyelid. There is no skin scar. It makes sense when the bag is fat and the skin does not hang, or hangs very little. On its own it does not allow removal of a marked skin fold.<\/p>\n\n<p><strong>Transcutaneous (subciliary) approach.<\/strong> The incision sits just below the lash line. It allows fat and skin to be treated in the same operation. A well-placed scar without tension is often not very visible, but it is a scar. It is not promised to be invisible on every skin and in every light.<\/p>\n\n<p>Choosing the route is an intraoperative decision prepared in consultation, not a marketing preference. If the skin is thin and the lid margin is lax, resecting skin \u201cjust in case\u201d is a poor idea. <a href=\"https:\/\/www.secpre.org\/pacientes\/cirugia-estetica\/parpados\" target=\"_blank\" rel=\"noopener\">SECPRE\u2019s patient information on eyelids<\/a> describes the procedure in general terms and recalls that the indication is individual. Also when a real excess of skin folds the lower eyelid and the person already understands what will not change.<\/p>\n\n<p>The indication should be postponed or declined if there is uncontrolled dry eye, active thyroid eye disease, an untreated allergy that swells the lid every week, or an expectation of changing the shape of the eye. Active smoking impairs healing; it is discussed in consultation, not turned into a slogan. Each case is decided by examination, not by an online questionnaire.<\/p>\n\n<h2><span class=\"ez-toc-section\" id=\"Recovery_without_a_marketing_calendar\"><\/span>Recovery, without a marketing calendar<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n<p>The first days bring swelling and, often, bruising. Sunglasses help with light and protection; they do not \u201cspeed up\u201d biology. Intense activity and effort that raises venous pressure in the face are resumed gradually, when examination allows it. Makeup is not a clinical milestone. Settling of the scar and the contour is judged over months, not in the first week.<\/p>\n\n<p>Possible complications include haematoma, infection, residual asymmetry, transient dry eye, lower-lid retraction and, uncommonly, a visual change that requires urgent review. Naming them is not a disclaimer: it is part of consent. A \u201cnatural\u201d result is not a guaranteed result. The aim is the best result possible for that anatomy.<\/p>\n\n<h2><span class=\"ez-toc-section\" id=\"Relation_to_other_surgery_of_the_gaze\"><\/span>Relation to other surgery of the gaze<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n<p>Yesterday\u2019s focus was the facelift and when it is indicated. Today\u2019s focus is only the lower eyelid. A facelift does not replace blepharoplasty, and blepharoplasty does not tighten the cheek or the neck. If descent of the midface is what marks the groove, the bag is sometimes not the main problem.
