lift: when it is indicated and what it does not correct

In consultation, one of the most frequent questions regarding the eyes is whether the problem lies in the eyelid or the position of the eyebrow. A brow lift is not a trimming of upper eyelid skin. It is a surgery that seeks to reposition the eyebrow when it has descended and that descent weighs on the eyelid or changes the expression. At Mallorca Medical Group, in Palma, Dr. José Ignacio García Ceballos, MD, FCCP and MRCS I England, assesses each case individually before indicating a procedure. The difference with eyelid surgery is explained on the page regarding blepharoplasty.

What is a brow lift

A brow lift, also called eyebrow elevation, is a facial plastic surgery intervention. Its objective is to place the eyebrow in a higher and more stable position when the descent is real, not just a perception from looking in the mirror under poor lighting.

It is not the same as an upper blepharoplasty. Blepharoplasty removes or redistributes skin and, in selected cases, eyelid bags. A brow lift acts on the position of the eyebrow and, depending on the technique, on the muscle and skin of the forehead. Confusing both procedures leads to operating on the wrong area.

Nor is it a treatment for expression wrinkles. Forehead and frown lines require a different approach. Mixing indications into a single promise of a “younger look” is not a clinical criterion.

When might it be indicated?

The indication does not come from a photo or a trend. It comes from the examination: eyebrow height at rest, symmetry, excess eyelid skin, frontalis muscle function, and the individual’s expectations.

True descent of the eyebrow that shortens the distance between the eyebrow and lashes and creates a feeling of heaviness, especially in the outer third.

Eyelid skin that appears to be excess because the eyebrow has lowered, not because the eyelid is the sole problem. In that case, removing only eyelid skin can leave the eyebrow even lower.

Stable asymmetry that the person wishes to partially correct, knowing that perfect symmetry is not a realistic goal.

Good general health and expectations aligned with what surgery can change: position, not personality or complete expression.

There are cases where it is not indicated. If the eyebrow is already high, raising it further can give a permanent look of surprise. If the problem is only thin eyelid skin or lower eyelid bags, a brow lift is not the tool. If there is eyelid ptosis (the edge of the eyelid covering the iris), a different diagnosis is needed, sometimes shared with ophthalmology.

What’s Changing and What Isn’t

What can change is the height and arch of the eyebrow, and with it the relationship between the eyebrow and the upper eyelid. In well-selected individuals, the gaze opens up without the need to remove as much eyelid skin.

What does not change is the shape of the eye, the color of the iris, lower eyelid bags, or dark circles due to pigment or hollows. Nor do all forehead lines disappear. And aging does not stop: the eyebrow may descend again over the years, to a degree that cannot be promised in advance.

The result depends on skin thickness, the strength of the frontalis muscle, previous surgeries, and how each person heals. Two similar foreheads do not yield the same postoperative experience.

Techniques, without a single recipe

There is no single technique valid for everyone. In consultation, it is explained which one fits and why, or why none fit yet.

Endoscopic approach, through small incisions in the scalp, for candidates with good elasticity and a descent that can be repositioned without removing much skin.

Hairline or coronal approach, when more control of the forehead skin is needed. The scar remains hidden in the hair, but the design matters for individuals with receding hair lines or thin hair.

Direct or temporal lift, in very selected cases, with a scar closer to the eyebrow or at the temple. It is not the first option if a visible scar can be avoided.

The choice is not a catalog for the patient to “choose the most modern one.” It is a decision made by the surgeon based on anatomy, hairline, and what needs to be preserved in the expression.

The consultation, before the operating room

At Mallorca Medical Group, the assessment is a medical consultation, not a no-cost visit or a fixed quote via message. Dr. García Ceballos reviews clinical photographs, the position of the eyebrow at rest and when raising the forehead, and the relationship with the eyelid. With 25 years of practice in plastic, reconstructive, and aesthetic surgery, and training in the British NHS (MRCS I England) and in plastic surgery in Brussels (FCCP), the criterion is also to say when not to operate.

Sometimes the answer is an upper blepharoplasty alone. Sometimes it is a brow lift. Sometimes it is waiting, or not indicating surgery. That distinction is the core of the consultation, not a detail.

Surgical activity is performed at Hospital Quirónsalud Palmaplanas and Hospital Quirónsalud Son Verí. The first conversation can be requested on the contact page. The framework for forehead and eyebrow surgery is found under facial surgery.

Recovery, in real terms

During the first few days, there is swelling in the forehead and eyelids, and sometimes bruising. Sleeping with the head elevated helps. The eyebrow may appear higher than it will ultimately sit: part of that height is edema, not the final result.

Scalp sutures, if any, are removed according to the technique, usually in the first or second week. Intense exercise and direct sun on scars are postponed until indicated by the surgeon. The stable shape of the eyebrow is judged in months, not weeks.

Discomfort, temporary asymmetry due to swelling, and a feeling of tightness in the forehead are common. Infection, a hematoma requiring drainage, or altered sensitivity are unusual but do occur. No brow lift is a procedure without risks, and no one can guarantee a specific result.

For whom this article is not an indication

This text does not replace an examination. It does not indicate surgery for those with high eyebrows, an unassessed eye disease, expectations of erasing age, or haste for an event in a few weeks. Nor does it compare clinics or prices: the cost is discussed in consultation, with the surgical plan at hand.

If the question is whether the weight of the gaze comes from the eyebrow or the eyelid, the answer lies in the assessment, not in an internet photo. At Mallorca Medical Group, that assessment is performed by Dr. García Ceballos, registered physician 070707779, at Camí Vileta 46C, Palma de Mallorca.

Medical review and editorial transparency

Medical review: Dr. García Ceballos, 070707779, specialist in plastic, reconstructive, and aesthetic surgery

Last review: 2026-10-25

This content has undergone human review and editorial control prior to publication.
