href="#Revision-medica-y-transparencia-editorial">Medical review and editorial transparency

Mastopexy, or breast lift, repositions the breast when the lower pole has descended and the nipple is below its ideal position. It is not a volume augmentation. This distinction is important because many consultations begin with the idea that lifting and filling are the same operation. They are not. Volume is decided separately, and an implant is not always necessary.

In Spain, ‘mastopexy’ maintains a stable search volume (around 12,100 monthly searches on Google Ads, Spanish market, October 2026 data). Interest does not equate to indication. What it does indicate is that the question is frequent: is native tissue sufficient, or is a prosthesis needed?

What Problem Does a Breast Lift Correct?

Breast ptosis is the descent of the nipple-areola complex and the lower pole relative to the inframammary fold. It can appear after pregnancies, lactation, weight loss, or due to aging and skin quality. During consultation, the nipple position, distance to the fold, residual volume, skin elasticity, and symmetry between both breasts are assessed.

A breast lift removes excess skin, repositions the nipple, and reshapes the glandular tissue. The expected result is a higher breast with improved shape. It is not a guarantee of cup size, perfect symmetry, or that the breast will not descend again with years, weight changes, or a new pregnancy.

When Native Tissue Is Sufficient

Mastopexy without an implant is appropriate when there is sufficient volume to fill the upper pole once the tissue is lifted. This is the case for many patients with ptosis and preserved glandular tissue who do not desire a larger size, or who prefer to avoid a foreign body.

In such cases, native tissue can be reorganized. One approach is to shift part of the lower pole upwards to provide projection to the décolletage, which in clinical practice is sometimes described as auto-prosthesis. Another is to combine the lift with a slight reduction if there is excess and the patient desires a smaller as well as higher breast. Neither of these ‘invents’ volume: if the glandular tissue is scarce, lifting without a prosthesis results in a firmer breast but not a fuller one.

Selection criteria include skin quality, vascularization, the relationship of the nipple to the fold, history of lactation or weight loss, and realistic expectations. If the goal is to restore a décolletage that native tissue cannot support, isolated mastopexy is not the correct indication.

When It Makes Sense to Add an Implant

An implant is considered when, after the lift, there would be insufficient projection or the patient desires a larger size than their glandular tissue allows. It is not an automatic solution or a standard ‘aesthetic bonus.’ It adds volume, but also a device that may require revision over the years, and different planning for the skin and pocket.

Combining a lift and an implant is more demanding than each procedure separately. The skin tension, implant position, and nipple vascularization must be balanced. In selected cases, Dr. García Ceballos applies the B.A.G.S. (Breast Anchoring Glandular Suspension) system, a proprietary glandular suspension technique designed to stabilize the upper pole in mastopexy, with or without an implant, in primary surgery and revisions. It does not replace the indication: it is used when the anatomy allows, not for all patients.

The Spanish Society of Plastic, Reconstructive and Aesthetic Surgery (SECPRE) describes breast lift as a procedure that repositions the breast and, if appropriate, can be combined with augmentation. This combination is decided during consultation, not from a form.

Scars and What Doesn’t Change

Lifting a breast with true ptosis leaves a scar. The pattern depends on the excess skin: periareolar for mild descents, vertical (around the areola and a line towards the fold) in many intermediate cases, and inverted T when the skin excess is greater. Promising a significant lift ‘without a scar’ is not anatomically supported.

What does not change with surgery: the underlying skin quality, the tendency for new pregnancies or weight changes, nor the possibility that, over the years, there may be a new partial descent. Future lactation depends on the technique and each individual case; it cannot be absolutely guaranteed or ruled out before surgery.

Recovery, in real terms

The first few weeks require a support bra, limiting arm elevation, and avoiding pectoral-moving exercises. Swelling gradually subsides. The final shape is not judged after one month: the breast continues to settle for several months. Scar care is part of the follow-up, not an accessory cosmetic detail.

As described in the general patient information from SECPRE, all surgery carries risks. In breast surgery, these may include: hematoma, altered nipple sensation, scarring problems, residual asymmetry, and, if an implant is present, contracture or the need for replacement. A proven safety profile is not the same as an absence of risk.

How Decisions Are Made in a Consultation

The decision does not come from a photo or a desired size. It comes from the examination: how much the nipple has descended, how much glandular tissue remains, how the skin behaves, and what result is reasonable for that anatomy. Dr. José Ignacio García Ceballos, MD, FCCP and MRCS I England, with 25 years of clinical practice and plastic surgery training at the Université Libre de Bruxelles (UZ Brussel, VUB), assesses at Mallorca Medical Group whether mastopexy alone is sufficient or if an implant provides something that native tissue cannot.

If native tissue is sufficient, adding an implant does not improve the result; it complicates it. If it is not sufficient, insisting on ‘lift only’ leaves a breast that is high but empty in the upper pole. That is the useful conversation, and it takes place during a personalized assessment, not as an offer.

To expand on the scope of breast surgery at the center, the breast surgery page includes related indications.
