rhinoplasty?<\/em> There is no universal answer, because the best technique is always the one that is precisely adapted to each patient&#8217;s individual anatomy and real goals. In this article I will explain, from the perspective of daily clinical practice, what distinguishes these two major trends in modern rhinoplasty and how we make this decision at Mallorca Medical Group. <\/p>\n<p><!-- imagen-destacada --><\/p>\n<h2><span class=\"ez-toc-section\" id=\"What_is_structural_rhinoplasty\"><\/span>What is structural rhinoplasty?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Structural rhinoplasty, also called open or classic technique with structural reconstruction, was for decades the gold standard in nasal surgery. In essence, it consists of resecting (removing) the nasal structures that generate the aesthetic or functional problem &#8211; bony and cartilaginous dorsum, tip, turbinates &#8211; and then reconstructing using cartilage grafts taken from the septum, auricular concha or even the ribs. <\/p>\n<p>This &#8220;take apart and rebuild&#8221; philosophy allows the surgeon total control over every millimeter of the nose. It is the gold standard technique when there are large deformities, previously scarred noses (secondary rhinoplasty), very thick skin requiring a strong cartilaginous framework, or when respiratory function is significantly compromised. The transcolumellar incision &#8211; the small scar in the columella under the nose &#8211; is its hallmark, and in expert hands it is virtually imperceptible after six months.  <\/p>\n<h2><span class=\"ez-toc-section\" id=\"What_is_rhinoplasty-preserving_rhinoplasty\"><\/span>What is rhinoplasty-preserving rhinoplasty?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Preservative rhinoplasty represents a profound philosophical shift: instead of resecting and reconstructing, the surgeon disimpacts, repositions and preserves the original nasal structures, especially Pitanguy&#8217;s ligament and the osteocartilaginous vault. The basic principle is that the nose already has its own architecture which, if respected, guarantees more natural results, minimal scarring and a shorter recovery. <\/p>\n<p>The preservation technique is gaining popularity throughout Europe, and for sound reasons. By keeping the dorsum intact &#8211; performing only repositioning and not resection osteotomies &#8211; it avoids the postoperative nasal step that sometimes impairs the results of the classic technique. In addition, by preserving the ligaments and supporting structures, the nasal tip retains its projection and rotation in a more stable manner in the long term. Recovery from edema is notably faster: many patients present a presentable result in 3-4 weeks, as opposed to the usual 6-8 weeks for the classic open technique.   <\/p>\n<blockquote><p>&#8220;Preservative rhinoplasty is not a fad: it is a surgical philosophy backed by solid scientific evidence. However, its application is not universal; it requires a precise selection of the patient and a real technical mastery. However, if there is a large deformity, bulbous tip with weak cartilage, secondary rhinoplasty or significant functional compromise, the structural technique &#8211; with autologous cartilage grafts &#8211; remains the best option. <\/p>\n<h2><span class=\"ez-toc-section\" id=\"The_role_of_the_surgeons_experience_and_training\"><\/span>The role of the surgeon&#8217;s experience and training<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Rhinoplasty preservation, despite its apparent conceptual simplicity, is technically demanding. It requires specific training in referral centers and a learning curve that few surgeons have rigorously completed. Claiming to practice the preserving technique without adequate training can lead to unsatisfactory results that are difficult to correct.  <\/p>\n<p>My training at the UZ VUB and ULB Brussels, as well as in the UK NHS, has enabled me to integrate the most current techniques in nasal surgery with sound clinical judgment.
