It does not rewrite the face, does not change skin thickness overnight, and does not replace an airway assessment if that was not the indication. Understanding this limit prevents expectations that surgery cannot meet.<\/p>\n<p>During consultation, the most useful question is not \u201cwill it be perfect?\u201d It is \u201cwhat part of this nose can be safely modified, and what part should be preserved?\u201d The answer depends on the nasal skeleton, the skin envelope, and whether there is an associated functional problem.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"What_Surgery_Can_Change\"><\/span>What Surgery Can Change<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>When indicated, rhinoplasty can adjust the dorsum, tip, alae, and the nose&#8217;s relationship with the upper lip. The change is structural: cartilage and, if necessary, bone are reshaped or repositioned. The visible result comes later, when the edema subsides. The initial contour is not the definitive contour.<\/p>\n<p>If there is nasal obstruction due to a deviated septum or another structural cause, a functional procedure can be considered during the same surgical time. Not every nose operated on for aesthetic reasons requires this procedure, and not every breathing difficulty is resolved by operating on the external shape. This behavior is not chosen in the operating room: it is measured and taken into account during planning.<\/p>\n<p>Nor does an undiagnosed respiratory alteration change on its own, nor does the subsequent aging of tissues. The operated nose continues to age with the rest of the face. And a stable result does not authorize promising millimeter symmetry: the original nose was already asymmetrical, and part of that asymmetry may persist.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"The_Skin_Envelope_Sets_the_Limit\"><\/span>The Skin Envelope Sets the Limit<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>Classifying the thickness of nasal soft tissues before surgery allows for adapting the technique and explaining to the patient what can be seen at twelve months. In thin skin, the priority is to avoid dorsal irregularities. In thick skin, the limit is in tip definition, not in \u201cthinning\u201d the skin with surgery.<\/p>\n<p>Dr. Jos\u00e9 Ignacio Garc\u00eda Ceballos, MD, FCCP, MRCS I England, has described a methodology to prevent dorsal irregularities in aesthetic rhinoplasty, published in Cirug\u00eda Pl\u00e1stica Ibero-Latinoamericana in 2024. The work, accessible in <a href=\"https:\/\/doi.org\/10.4321\/s0376-78922024000400005\" target=\"_blank\" rel=\"noopener\">this CIRPLAS article<\/a>, stratifies skin thickness and describes the intraoperative use of nanograft enriched with L-PRF in selected cases. It is a protocol from a clinical series, not a guarantee for every patient.<\/p>\n<h2><span class=\"ez-toc-section\" id=\"For_Whom_It_Makes_Sense_and_For_Whom_It_Doesnt\"><\/span>For Whom It Makes Sense, and For Whom It Doesn&#8217;t<span class=\"ez-toc-section-end\"><\/span><\/h2>\n<p>It makes sense to consider rhinoplasty when there is a specific anatomical alteration\u2014dorsum, tip, alae, or airway\u2014and the patient understands that the change is progressive.
