The myth that “blepharoplasty leaves a surgical look” usually stems from a result in which more skin or fat was removed than the eyelid needed, or from an indication that did not distinguish the eyelid from the eyebrow. The clinical goal is to relieve the excess that weighs on the eyes and preserve the shape of the eye.

During consultation, Dr. José Ignacio García Ceballos, MD, FCCP and MRCS I England, Director of Mallorca Medical Group, reviews each case individually. After 25 years of practice in plastic, reconstructive and aesthetic surgery, what is assessed is not an eyelid “template”, but the relationship between skin, fat, lateral canthal support, eyebrow position and the tear film. What you see at two weeks is not the final result.

Where the surgical look comes from

An upper eyelid that looks too hollow, an artificially high crease, or a lower eyelid that sits away from the eyeball are not “the normal look after blepharoplasty”. They usually correspond to excessive resection, failure to support the lateral canthus when the eyelid was already lax, or operating on the eyelid when the main descent was in the eyebrow. The Spanish Society of Plastic, Reconstructive and Aesthetic Surgery (SECPRE) reminds us that this surgery removes excess skin and, when indicated, fat or muscle, and that it does not by itself correct crow’s feet or eyebrow droop.

Nor is everything interpreted as “surgical” actually surgical. Swelling, mild asymmetry and a more open look in the first weeks are part of inflammation. That appearance changes. The result should be assessed at rest and in motion, months later—not in the day-seven photo.

What can change and what does not change

Upper blepharoplasty can reduce excess skin that folds over the eyelid and, in selected cases, improve the visual field when that excess limits it. Lower blepharoplasty can reduce fatty bags that bulge the eyelid. Neither guarantees perfect symmetry, neither eliminates expression lines in the periocular area, and neither replaces a brow lift or a facelift if the issue is higher up or in the midface.

The surgical approach also matters. In the lower eyelid, the transconjunctival approach avoids a skin scar when the issue is mainly fat and there is no excess skin. If there is true skin excess, a transcutaneous approach may be needed, with conservative resection. The clinical information sheet on blepharoplasty at Mallorca Medical Group distinguishes upper, lower and combined precisely because they are not the same indication. The surgical technique is detailed in this guide to the blepharoplasty technique, without replacing an in-person assessment.

How expression is protected

Preserving expression is not a promise of outcome. It is a planning criterion. In Dr. García Ceballos’ practice, the amount of skin removed is decided with the eye closed and with the patient in a position that does not artificially tighten the eyelid. Removing “a little more just in case” is one of the most common causes of a hollow eyelid or incomplete closure.

In the lower eyelid, the risk that most affects appearance is ectropion: the eyelid margin pulls away from the eye. For this reason, before surgery, canthal laxity and tear film quality are assessed. If there is dry eye, thyroid disease, or already limited eyelid closure, surgery may not be indicated, or may be indicated only partially. Each patient is an individual case. Not every eyelid with bags should be operated on.

Fat is not routinely removed either. In some upper eyelids, a tired look comes from a hollow groove, not from excess. Removing that fat worsens the appearance. In the lower eyelid, it may sometimes be advisable to restore part of the volume in the groove, not only resect it. That decision is made in the operating theatre based on what is found, not on a template.

Who may be a candidate and who may not

The typical candidate is an adult in good general health, with demonstrable skin or fat excess, realistic expectations, and sufficient eyelid closure. A person is not a candidate, or is a candidate with reservations, if they seek to erase all wrinkles around the eye, have uncontrolled dry eye, or attribute to the eyelids a descent that actually corresponds to the eyebrow. The consultation also serves to say when not to operate.

Recovery varies. Bruising and a feeling of tightness are common during the first few days. Stitches, when present, are usually removed around the first week. The final shape of the crease settles over months. During that time, sun protection and follow-up are part of the treatment, not an optional extra.

What should be clarified before deciding

A useful assessment includes photographs at rest and with gaze, examination of the eyebrow, a canthal laxity test, and questions about ocular dryness, allergy, thyroid disease, and medication that affects clotting. Based on this assessment, upper blepharoplasty, lower blepharoplasty, combined blepharoplasty, an associated brow procedure, or no procedure may be indicated.

Mallorca Medical Group sees patients at Camí Vileta 46C, Palma de Mallorca. The consultation is not a commercial formality: it is the moment when surgery is accepted or ruled out.
