Breast Asymmetry: Causes, Diagnosis and Surgical Solutions in Majorca

Breast asymmetry, known in medical terminology as anisomastia, is one of the most common consultations in the field of breast plastic surgery. It is defined as the appreciable difference in volume, shape, position or arrangement of the nipple-areola complex between the two breasts. Although a slight difference between the two breasts is a frequent anatomical variant in the female population, when the asymmetry is notorious and conditions the quality of life, self-esteem or clothing selection, the evaluation with a specialist in plastic surgery allows determining the most appropriate therapeutic options for each case.

At Mallorca Medical Group, Dr. García Ceballos FCCP, a plastic surgeon with reference training at the Université Libre de Bruxelles and the UZ VUB in Brussels, approaches the study and correction of breast asymmetry with an individualized approach, aimed at obtaining natural and proportionate results while respecting the morphological characteristics of each patient. This guide offers rigorous medical information on the causes, diagnosis and correction alternatives available for breast asymmetry in the context of Plastic Surgery in Mallorca.

What is breast asymmetry and how often does it occur?

Breast asymmetry encompasses a broad spectrum of clinical situations. It can present as a difference in volume between the two breasts, a discrepancy in the position of the inframammary fold, a variation in the size or location of the areola-nipple complex, or a combination of several of these alterations.

From the clinical point of view, various degrees of asymmetry can be distinguished:

Mild grade: a volume difference of less than one bra cup size, with minimal functional or aesthetic impact.

Moderate grade: a difference of one cup size or more, affecting the choice of clothing and lingerie.

Severe grade: a marked difference that may be associated with morphological alterations such as tuberous breast or chest wall asymmetries.

Clinical statistics indicate that most women present some degree of natural breast asymmetry. However, in a medical context, correction is considered when the difference is objectively significant and causes discomfort to the patient.

Most common causes of breast asymmetry

The origin of breast asymmetry is multifactorial. The main causes identified during clinical assessment include:

Asymmetric pubertal development: differences in the response of glandular tissue to hormonal stimuli during puberty.

Congenital variations: including tuberous breast, Poland syndrome, and other morphological alterations present from birth.

Post-pregnancy and post-breastfeeding changes: pregnancy and breastfeeding can accentuate or create differences between the two breasts.

Chest wall asymmetry: variations in the rib cage or spine can affect breast anatomy.

History of breast surgery: previous surgeries, biopsies, or procedures on the breast can lead to secondary asymmetries.

Significant weight changes: marked weight variations affect each breast differently.

The precise identification of the origin of the asymmetry is a fundamental step to guide the surgical plan, since not all asymmetries are corrected in the same way and do not require the same technique.

Diagnostic assessment: personalized evaluation as the starting point

The diagnostic process for breast asymmetry at Mallorca Medical Group begins with a thorough clinical assessment. This includes a detailed patient history, a bilateral physical examination of the breast, and a morphological analysis of the following parameters:

Volume of each breast and the difference between them.

Position of the right and left inframammary folds.

Degree of bilateral breast ptosis.

Morphology, size, and position of the areola-nipple complex.

Characteristics of the skin envelope and glandular tissue.

Analysis of the underlying chest wall.

“The correction of breast asymmetry requires a detailed analysis of each breast independently and in relation to the patient’s overall body morphology. There is no single protocol: each surgical plan is designed according to the individual clinical findings and the morphological objective agreed with the patient.”

– Dr. García Ceballos FCCP, plastic surgeon at Mallorca Medical Group

In certain cases, the exploration is complemented with imaging tests – mammary ultrasound or mammography – to obtain additional information about the glandular tissue before planning the intervention.

Surgical correction options for breast asymmetry

The choice of surgical technique is made on an individual basis depending on the type and degree of asymmetry, each patient’s morphological characteristics, and their expectations. The main options available in breast surgery for treating asymmetry include:

Type of asymmetry

Usual surgical options

Volume difference (smaller breast)

Breast augmentation with implants or lipofilling

Volume difference (larger breast)

Breast reduction or reduction mastopexy

Difference in ptosis

Unilateral or bilateral asymmetric mastopexy

PDA asymmetry

Areoloplasty, nipple repositioning

Combined asymmetry

Combined technique in the same surgical act

Post-surgical asymmetry

Secondary or revision breast surgery

In cases in which breast augmentation with implants is considered, the selection of the implant -volume, profile, type of surface and plane of placement- is made taking into account the anatomical differences between both breasts to obtain a symmetrical and proportionate result. In some patients with moderate differences in volume, lipofilling or breast lipofilling can be a suitable alternative that avoids the use of implants and takes advantage of the patient’s own autologous fat.

Recovery and realistic expectations after breast asymmetry surgery

Recovery after breast asymmetry surgery varies depending on the extent of the procedure performed. In general terms, the most relevant aspects of the postoperative period include:

First 48-72 hours: relative rest, use of a support bra, and adherence to the prescribed pain management protocol.

First week: medical follow-up visit to assess healing progress and the condition of the drains, if used.

Weeks 2-4: gradual return to low-intensity daily activities.

1-3 months: stabilization period for the morphological result. Postoperative swelling and firmness gradually resolve.

6-12 months: final result becomes apparent, with complete scar maturation.

It is important that patients maintain realistic expectations regarding the outcome of the correction. The aim of the intervention is to achieve a significant improvement in symmetry, but absolute anatomical perfection is not always achievable. Continued postoperative follow-up at Mallorca Medical Group guarantees medical support throughout the recovery process.

Trusted breast asymmetry and plastic surgery in Mallorca

Choosing a plastic surgeon with accredited training and proven experience is a determining factor in the safety and results of any breast procedure. In the context of Aesthetic Surgery in Mallorca, Dr. García Ceballos FCCP brings together six years of training in reconstructive and aesthetic plastic surgery at Belgian university hospitals, FCCP accreditation from the Collegium Chirurgicum Plasticum of Belgium, and more than three decades of active clinical experience.

Mallorca Medical Group operates in the surgical facilities of Hospital Quirónsalud Palmaplanas and Hospital Quirónsalud Son Verí, guaranteeing clinical safety protocols typical of a Trusted Plastic Surgery Clinic in Spain. Reconstructive Surgery in Mallorca, including the correction of secondary or congenital breast asymmetries, is part of the specialized activity of Dr. García Ceballos within Mallorca Medical Group.

The indication of an intervention for the correction of breast asymmetry, as well as the estimation of the cost of plastic surgery in Mallorca for this type of procedure, requires a previous face-to-face clinical evaluation. The cost varies depending on the complexity of the asymmetry, the technique selected and the type of surgical facility used.

Frequently asked questions about breast asymmetry

Is it normal to have one breast larger than the other?

A slight difference in volume or shape between the two breasts is a common anatomical variation and does not necessarily indicate a pathology. It is estimated that most women present some degree of natural breast asymmetry. However, when the difference is visually noticeable, causes discomfort, or affects the choice of clothing and lingerie, a medical evaluation may be considered to explore correction options. Consulting a specialized plastic surgeon allows for determining the degree of asymmetry and the most appropriate treatment plan for each case.

What are the most frequent causes of breast asymmetry?

The causes of breast asymmetry are many and varied. Among the most frequent are differences in pubertal development, which can lead to different volumes between the two breasts; variations in the glandular tissue or in the arrangement of the nipple-areola complex; asymmetries of the underlying chest wall; post-gestational changes or after breastfeeding; and, less frequently, a history of previous breast surgery. In some cases anisomastia may be associated with morphologic alterations such as tuberous breast or congenital conditions. Individualized medical evaluation is essential to identify the origin of the asymmetry.

What surgical techniques are used to correct breast asymmetry?

Surgical correction of breast asymmetry depends on the type, degree and specific cause of the asymmetry. The most common options include breast augmentation with implants (on the side with less volume), breast reduction or mastopexy (on the side with more volume or greater ptosis), the combination of both techniques in the same procedure, and breast lipofilling or lipofilling for moderate volume corrections. In cases of asymmetry of the nipple-areola complex, an areoloplasty can be performed. The selection of the appropriate technique is made on an individual basis after the morphological and functional assessment of each patient.

When is surgical intervention recommended for breast asymmetry?

The surgical indication is assessed on an individual basis, depending on the extent of the asymmetry, its impact on the patient’s quality of life, and the absence of medical contraindications. In general, it is advisable to wait until breast development is complete (usually after age 18) before planning a definitive procedure. Situations such as marked asymmetry following previous breast surgery, asymmetry associated with breast pathology, or significant morphological variations causing psychological distress are common indications for specialized assessment.

Round vs. Anatomical Breast Prostheses: Complete Clinical Guide by Dr. Garcia Ceballos FCCP

Author: Dr. José Ignacio García Ceballos FCCP – Plastic, Aesthetic and Reconstructive Surgeon – Mallorca Medical Group

One of the most relevant decisions in any breast augmentation process is not the size of the implant, but its morphology. Choosing between a round and an anatomical prosthesis directly influences the final appearance of the breast, the naturalness of the result and the long-term safety. However, this choice cannot be made without an individualized clinical assessment. There is no universally superior implant: there is the most suitable implant for each patient and for each specific case.

In this guide, Dr. José Ignacio García Ceballos FCCP, a plastic surgeon with 34 years of clinical practice and specialized training at UZ VUB Brussels, explains the fundamental differences between the two types of implants, their clinical indications, advantages, limitations and the criteria that determine the choice in each case.

Implant morphology: what differentiates round implants from anatomical implants?

The most obvious difference between the two types of prostheses lies in their geometry. Round implants present a symmetrical silhouette in all directions and distribute the volume evenly, with special presence in the upper pole of the breast. This generates an effect of greater filling in the highest area of the cleavage.

Anatomical implants, also called teardrop implants, reproduce the natural silhouette of the breast: greater volume in the lower pole and a progressive and smooth transition to the upper pole. This profile is closer to the physiological breast anatomy, especially in small breasts with little breast tissue.

From a technical point of view, anatomical implants require greater precision in the dissection of the periprosthetic pocket, since their asymmetry makes it essential that they remain in the correct orientation. Round implants do not present this condition, since their radial symmetry makes any rotation inside the pocket irrelevant.

Clinical criteria for implant selection

The choice of the type of prosthesis should be based on a thorough analysis of the morphology of each patient. The main factors that Dr. Garcia Ceballos evaluates in consultation are the following:

Amount of pre-existing breast tissue: in patients with little tissue of their own, the anatomical prosthesis can provide a more natural look by avoiding excess volume in the upper pole.

Breast ptosis: the sagging of the breast tissue determines the type of implant and, in many cases, the need to combine augmentation with mastopexy.

Width and height of the breast base: the measurement of the base of the breast (base width) is decisive in selecting the diameter of the implant, regardless of its morphology.

Chest morphology: the shape of the chest, the position of the submammary folds and the inter-nipple distance determine both the morphology and the profile of the implant.

Aesthetic expectations of the patient: the desired result – more natural and physiological, or with greater definition of the cleavage – largely determines the choice.

Placement plane: subglandular, subfascial, submuscular or dual plane. The interaction between the plane and the implant morphology influences the final result.

“No implant is better in the abstract. The correct prosthesis is the one that adapts to the real anatomy of that particular patient, not the one that is in fashion or the one that most patients have asked for that week. The surgeon’s job is to find the solution that combines aesthetic result, safety and long-term durability.”

Advantages and limitations of round implants

Round implants are the most widely used worldwide and have a well-documented safety and efficacy profile. Their main clinical advantages are:

Greater flexibility in terms of intracapsular rotation: as they are symmetrical, any displacement does not alter the visible result.

Wide variety of profiles (low, moderate, high and ultra-high), allowing precise customization of the result.

Surgical technique of simpler and more predictable placement.

Particularly favorable result in patients who desire greater projection and definition of the upper pole of the cleavage.

However, in patients with very scarce breast tissue, excess volume in the upper pole can create an unnatural or artificial appearance, which is precisely what many patients wish to avoid. In these cases, the anatomical implant may be the most indicated option.

Advantages and limitations of anatomical implants

Anatomical implants have gained clinical prominence in recent decades, especially in Europe, where the search for natural results is a widespread priority among patients. Their main advantages are:

Volume distribution closer to physiological breast anatomy.

Particularly natural result in patients with scarce breast tissue and small breasts with little ptosis.

Less appearance of excessive “roundness” in the upper pole.

Profile suitable for breast reconstruction after mastectomy, where the aim is to restore a physiological silhouette.

Its main limitation is the risk of rotation if the periprosthetic pocket is not precisely designed. A rotation of the anatomical implant generates a visible asymmetry that may require surgical correction. This risk is minimized with meticulous technique and correct pocket sizing, but cannot be completely eliminated.

Likewise, the texturization of the anatomic implant surface – necessary to reduce the risk of rotation – has been subject to regulatory review in recent years. It is important that the patient is informed about the characteristics of the proposed implant surface and the current scientific evidence on the subject.

Next-generation implants: beyond round vs. anatomical

The breast implant industry has evolved significantly in the last decade. Today, the strict classification between “round” and “anatomical” coexists with new categories that expand the possibilities for customization:

Ergonomic implants: designed to adapt to the patient’s posture, dynamically shifting the volume depending on whether the woman is standing or lying down.

Nanotextured and smooth implants: alternatives that reduce the chronic inflammatory reaction and the risk of capsular contracture with respect to rougher surface implants.

High density cohesive gel: highly cohesive gel implants (colloquially called “gummy bear implants”) maintain their shape even in the event of a rupture of the outer shell, increasing the safety of the result.

Hybrid breast augmentation: combination of implant and autologous fat lipoinjection to customize the result with greater anatomical precision.

The selection within these categories is also part of the personalized preoperative assessment and depends on the individual characteristics of each patient.

Preoperative planning in breast surgery by Dr. Garcia Ceballos

Dr. Garcia Ceballos performs a structured preoperative assessment that includes biometric measurement of the breast base, analysis of the skin and subcutaneous tissue, evaluation of the submammary fold and clinical photographic documentation. When indicated, it is complemented with digital simulation of the result to facilitate mutual understanding between surgeon and patient.

This planning is not intended to guarantee a specific outcome – something no honest surgeon can promise – but to establish the framework that maximizes the likelihood of obtaining the best possible result for that particular patient and that particular anatomy. Individual biological variability, scar response and the natural evolution of breast tissue over time are factors that must always be considered within a context of realistic expectations.

Dr. García Ceballos operates in the operating rooms of Hospital Quirónsalud Palmaplanas and Hospital Quirónsalud Son Verí, centers with the certifications and safety standards required for high complexity plastic surgery.

Frequently asked questions about round vs. anatomical breast prostheses

What is the main difference between a round and an anatomical prosthesis?

The round prosthesis has the same curvature over its entire surface and provides greater volume in the upper pole of the breast. The anatomical prosthesis, teardrop-shaped, reproduces the natural silhouette of the breast with more volume in the lower pole and a smooth transition to the upper pole. The choice depends on the patient’s body morphology, the existing volume of the breast and the desired aesthetic result.

Can anatomical implants rotate and what are the consequences?

Anatomical implants, having an asymmetric shape, can rotate inside the periprosthetic pocket if the surgical technique is not precise. A rotation alters the visible shape of the breast. For this reason, their placement requires a meticulous surgical technique and a correctly sized pocket. Round implants do not present this risk since their radial symmetry makes any rotation go unnoticed.

What type of prosthesis is most suitable for patients with little breast tissue of their own?

In patients with scarce or very thin breast tissue, the anatomical prosthesis can offer a more natural result by distributing the volume progressively, avoiding the appearance of excessive filling in the upper pole. However, each case must be evaluated individually in consultation, also considering the access route, the plane of placement and the patient’s expectations.

How long is the recovery time after breast augmentation surgery with prostheses?

Initial recovery after breast augmentation covers the first 48-72 hours, during which there may be moderate discomfort controllable with analgesia. The return to sedentary activities is usually possible between the fifth and seventh day. Intense physical exercise and efforts with the upper body are resumed progressively after four to six weeks, following the surgeon’s postoperative follow-up protocol.
