Blepharoplasty corrects excess skin and fat deposits on the upper and lower eyelids. Its goal is not to “change the look” but to restore a fresh, rested appearance that respects the patient’s natural expression.

At Mallorca Medical Group, we routinely incorporate a specialized technique in lower blepharoplasty: L-PRF-enriched nanofat grafting applied to the cheekbone-eyelid junction. This prevents the “hollow eye” effect or “skeletalization” of the infraorbital arch, a relatively common aesthetic complication in conventional lower blepharoplasty procedures.

Ideal candidates

Patients aged 40–45 and older with excess skin on the upper eyelid (which may limit the field of vision), lower eyelid fat pads, a pronounced tear trough, or structural dark circles under the eyes. Also, younger patients with hereditary lower eyelid fat pads.

02 — Types of Blepharoplasty

A technique for each eyelid

Upper Blepharoplasty

Removal of excess skin from the upper eyelid. When there is a significant amount of excess skin, it can limit the field of vision (functional aspect). The scar is hidden in the eyelid crease and is imperceptible once healed.

Combined Blepharoplasty

Combined upper and lower eyelid surgery performed in a single procedure. This optimizes recovery and provides overall rejuvenation of the periorbital area.

Blepharoplasty + canthopexy / cantoplasty

When there is laxity of the lower eyelid, tightening of the outer canthus is performed to prevent postoperative ectropion. This is particularly indicated in patients over 60 years of age or who have undergone previous surgery.

Nanofat + L-PRF: Why We Routinely Combine Them

Nanofat is the patient’s own fat that has been processed into a very fine emulsion rich in regenerative factors. When combined with L-PRF (fibrin-rich plasma obtained by centrifugation), it improves the quality of the periorbital skin and naturally fills the infraorbital groove. This is what distinguishes modern lower blepharoplasty from a classic “draining” technique.

Week by Week After Blepharoplasty

    Rest with local cold therapy

    Relative rest, head elevated, and intermittent application of cold to the affected area. Moderate periorbital bruising, sensation of eyelid tightness.

  • D5-7

    Removal of sutures

    Stitches removed from the upper eyelid; absorbable sutures in the lower eyelid. Bruising subsiding. Partial return to normal activities.

  • S2-3

    Resolved bruises

    Any residual bruising can be covered with makeup. Full return to social activities. Continue using artificial tears.

  • S4-6

    Mature Scars

    The scars along the eyelid crease become imperceptible. The results are clearly visible.

    Final result
