A technique for every patient and every situation
Two-stage expander + implant procedure
The standard technique when radiation therapy has not been performed or is not planned. A tissue expander is placed and gradually inflated in the clinic until the desired volume is reached; in a second stage (3–6 months), it is replaced with the permanent implant.
Direct Implant
Placement of the permanent implant in a single procedure, often using an acellular dermal matrix to improve coverage. Indicated when the skin and muscle allow it and no radiation therapy is planned.
DIEP flap (autologous tissue)
A microsurgical procedure that transfers skin and fat tissue from the abdominal wall to the breast without removing any muscle. This technique produces the most natural and long-lasting results and is especially recommended for patients who have undergone radiation therapy or who prefer to avoid implants. It requires a higher level of technical expertise.
Gradual Lipofilling
Reconstruction through successive sessions of autologous fat transfer. Suitable for partial mastectomies or as a complement to other techniques. It allows for the refinement of contours and improves the quality of irradiated skin.
Hybrid reconstruction (implant + lipofilling)
Combine a small or medium implant with lipofilling to refine the contour, improve the axillary transition, and achieve a natural feel. This approach is increasingly used as a compromise between simplicity and naturalness.
Areola and Nipple Reconstruction
Final phase of reconstruction. This can be performed using 3D tattooing (an imperceptible, non-surgical technique) or through surgical reconstruction using local flaps. It is usually performed 3–6 months after the primary reconstruction.

Postoperative Care and Timelines

Initial Hospitalization
1–4 nights in the hospital, depending on the procedure (DIEP requires a longer stay for flap monitoring). Drains in the reconstructed area and, in the case of DIEP, also in the abdomen.

Removal of drains
Gradual removal of drains during the follow-up visit. Wear a sports bra 24 hours a day. Limit arm movement to gentle motions only.

Partial Reinstatement
Return to light daily activities. Do not raise your arms above your head. Patients who have undergone DIEP surgery should take it easier due to the abdominal scar.

Progressive Active Life
Resumption of sedentary work activities. Gentle aerobic exercise. Specific physical therapy if indicated (preventive treatment for lymphedema).

Second half (if applicable)
In two-stage reconstructions: replacement of the expander with a permanent implant. Planning for contralateral symmetry, if applicable.
