In the world of nasal surgery, few questions generate as much debate among patients and surgeons as this one: structural or conservative rhinoplasty? There is no universal answer, because the best technique is always the one that is precisely adapted to each patient’s individual anatomy and real goals. In this article I will explain, from the perspective of daily clinical practice, what distinguishes these two major trends in modern rhinoplasty and how we make this decision at Mallorca Medical Group.
What is structural rhinoplasty?
Structural rhinoplasty, also called open or classic technique with structural reconstruction, was for decades the gold standard in nasal surgery. In essence, it consists of resecting (removing) the nasal structures that generate the aesthetic or functional problem – bony and cartilaginous dorsum, tip, turbinates – and then reconstructing using cartilage grafts taken from the septum, auricular concha or even the ribs.
This “take apart and rebuild” philosophy allows the surgeon total control over every millimeter of the nose. It is the gold standard technique when there are large deformities, previously scarred noses (secondary rhinoplasty), very thick skin requiring a strong cartilaginous framework, or when respiratory function is significantly compromised. The transcolumellar incision – the small scar in the columella under the nose – is its hallmark, and in expert hands it is virtually imperceptible after six months.
What is rhinoplasty-preserving rhinoplasty?
Preservative rhinoplasty represents a profound philosophical shift: instead of resecting and reconstructing, the surgeon disimpacts, repositions and preserves the original nasal structures, especially Pitanguy’s ligament and the osteocartilaginous vault. The basic principle is that the nose already has its own architecture which, if respected, guarantees more natural results, minimal scarring and a shorter recovery.
The preservation technique is gaining popularity throughout Europe, and for sound reasons. By keeping the dorsum intact – performing only repositioning and not resection osteotomies – it avoids the postoperative nasal step that sometimes impairs the results of the classic technique. In addition, by preserving the ligaments and supporting structures, the nasal tip retains its projection and rotation in a more stable manner in the long term. Recovery from edema is notably faster: many patients present a presentable result in 3-4 weeks, as opposed to the usual 6-8 weeks for the classic open technique.
“Preservative rhinoplasty is not a fad: it is a surgical philosophy backed by solid scientific evidence. However, its application is not universal; it requires a precise selection of the patient and a real technical mastery. At Mallorca Medical Group, we indicate it when the patient’s anatomy favors it, not as a response to a market trend.“

Key differences between the two techniques
The following table summarizes the most relevant clinical criteria for choosing between one technique and the other:
What technique is used at Mallorca Medical Group?
A patient with a moderate nasal hump, normal skin thickness and a nose with no previous surgery is usually an excellent candidate for the conservative technique. However, if there is a large deformity, bulbous tip with weak cartilage, secondary rhinoplasty or significant functional compromise, the structural technique – with autologous cartilage grafts – remains the best option.
The role of the surgeon’s experience and training
Rhinoplasty preservation, despite its apparent conceptual simplicity, is technically demanding. It requires specific training in referral centers and a learning curve that few surgeons have rigorously completed. Claiming to practice the preserving technique without adequate training can lead to unsatisfactory results that are difficult to correct.
My training at the UZ VUB and ULB Brussels, as well as in the UK NHS, has enabled me to integrate the most current techniques in nasal surgery with sound clinical judgment. The publication of my research on rhinoplasty and L-PRF nanograss in SciELO (2024) reflects my commitment to evidence-based innovation. This combination – international training, intense clinical practice and continuous scientific updating – is what guarantees safe and natural results in every case.
Frequently Asked Questions (FAQs)
- Can any surgeon perform a conservative rhinoplasty?
- No. Preservative rhinoplasty requires specific training and involves a significant learning curve. It is essential to choose a plastic surgeon with proven experience in both techniques so that they can offer you the one best suited to your case—not the one they are most skilled at or the one that is currently in vogue.
- Does rhinoplasty-preserving rhinoplasty give more natural results?
- In well selected cases, yes. By preserving the original nasal structures, the result better respects the natural proportions of the nose and avoids the “operated appearance”. However, in expert hands, the structural technique can also achieve very natural-looking results.
- How long is the recovery time for each technique?
- Preservative rhinoplasty usually offers a faster recovery: the main edema resolves in 3-4 weeks. In the open structural technique, the definitive result may take 12-18 months. In both cases, normal social activity can be resumed in 7-10 days with appropriate medication and care.
- Can I know in advance which technique I need?
- Only after a complete face-to-face medical evaluation. In our office we use 3D digital simulation and photographic analysis to help you visualize the possible results before making any decision. Book your personalized assessment at mallorcamedicalgroup.com and we will offer you a tailor-made surgical plan.
Conclusion
The structural vs. preserving rhinoplasty dichotomy is not a debate of “which one is better”, but of “which one is right for each patient”. At Mallorca Medical Group, our philosophy is always the same: the technique at the service of the result, not the other way around. If you are considering nasal surgery in Mallorca and want an expert and independent opinion, I invite you to know your case.
