Define the cheekbones and contour the lower third of the face

A buccal fat pad removal involves the controlled partial removal of the Bichat fat pads—two deep fat deposits located in the cheeks. Reducing these deposits visually slims the lower third of the face and highlights the prominence of the cheekbones, creating a more refined appearance.

It is a simple surgery, but it requires careful selection of candidates: if performed on thin faces or if too much tissue is removed, it can lead to premature aging (“skull-like face”) within 10–15 years. Proper indication is essential.

Ideal candidates

Patients between the ages of 25 and 45 with a round face, full cheeks, and excess fatty tissue in the cheeks despite a stable body weight. It is NOT indicated for very thin patients, those over 50, or those with little facial fatty tissue.

It's not for everyone

Bichectomy + Cheek Lipofilling

When the goal is to define the cheekbones and refine the lower third of the face, this can be combined with a small amount of lipofilling in the cheekbones to enhance facial harmony.

Not recommended in these cases

Very thin patients, those with already sculpted faces, those over 50, or those with little facial fat. In these cases, buccal fat removal would lead to premature aging (“skull-like face”) in the medium term.

Non-surgical alternatives

For patients with unclear indications, non-surgical alternatives are available: deoxycholic acid, facial cryolipolysis, or thread lift.

The most common mistake

Performing buccal fat removal on poorly selected patients (those with thin faces, over 50). The short-term result may appear favorable, but after 10–15 years, the face ages prematurely. Strict adherence to indications is what distinguishes a well-performed buccal fat removal from a poorly performed one.
