Restoring the Upper Arm Contour After Weight Loss
Brachioplasty is a surgical procedure that removes excess skin and fat from the inner side of the arm. It is primarily indicated for patients who have lost a significant amount of weight—whether following bariatric surgery, GLP-1 therapy (Ozempic, Wegovy), or an intensive diet—and in whom the skin of the upper arm does not retract on its own, leaving a loose skin flap colloquially known as “wings” or “bingo wings.”
The rise in GLP-1 use has led to a significant increase in the number of patients requiring post-weight-loss surgery. Brachioplasty is one of the most commonly requested procedures in this segment, along with post-bariatric abdominoplasty and cruroplasty.
Ideal candidates
Patients whose weight has remained stable for at least 6–12 months following weight loss, who are in good nutritional status, are nonsmokers, and have realistic expectations regarding the arm scar (which is visible, though subtle). Also, patients with arm sagging due to skin aging or body type.

Adapt the technique to the degree of sagging
Isolated brachial liposuction
When there is only excess fat and the skin is elastic (young patient with stable weight). No visible scarring, quick recovery. NOT recommended if there is skin laxity.
Axillary mini-brachioplasty
Skin excision limited to the upper third of the arm, with a scar hidden in the axillary crease. Recommended for mild, localized sagging.
Standard Brachioplasty with Liposuction
Vertical skin resection from the armpit to near the elbow, combined with liposuction to refine the contour. The standard technique for moderate to severe skin laxity following weight loss.
Extended Brachioplasty
For severe post-bariatric skin laxity: the incision extends laterally to the trunk, also treating the lateral thoracic area. A longer incision but better results in extreme cases.
Unlike other surgeries, the axillary scar is visible (on the inner side of the arm; visible when the arm is raised). This is the main drawback of the technique. Once it has healed (12 months), it becomes less noticeable, but the patient must accept it as a trade-off for the aesthetic benefit.
Week by Week After Brachioplasty

Vendaje compresivo
Discharged the same day or the next day. Compression bandage for 48 hours. Arms elevated with a pillow. Moderate pain controlled with oral analgesics.

Removal of sutures
Wear a compression sleeve during the day. Stitches are removed on day 10–14. Return to sedentary work.

Moderate physical activity
Walking and a sedentary lifestyle. No weight on the arms. Continuous compression bandage.

Progressive Sports
Light cardio. No specific arm exercises. The scar is starting to heal.
- M3-12
Permanent Scar
The scar heals gradually: red after 3 months, pink after a year. Strict sun protection for 12 months on the scar.
