Rhinoplasty is one of the most requested interventions in plastic surgery, but also one of the most challenging. A frequent problem is the appearance of small irregularities in the nasal dorsum after surgery, especially in patients with thin skin. To address this complication, Dr. José Ignacio García Ceballos, MD, has developed an innovative method in the article in the latest issue of the IBERO-LATINOAMERICAN PLASTIC SURGERY JOURNAL.

TheThin Nasal Skin Challenge
In patients with thin skin, the nasal dorsum may be left with visible irregularities after surgery. This occurs because the underlying bones and cartilage are not sufficiently covered, which can result in sharp edges or asymmetries. While there are classic techniques to manage this problem, they often do not offer entirely satisfactory solutions for those with a particularly delicate nasal dorsum.
A Prevention-Based Strategy
Dr. Garcia Ceballos proposes a simple but effective approach: measuring nasal skin thickness before surgery. Using a hand-held adipometer, he classifies patients into two types:
- Type I: Thin skin (<3 mm), with greater risk of irregularities.
- Type II: Thicker skin (≥3 mm), with less likelihood of complications.
Based on this classification, adapt the treatment:
- In Type I patients, he applies leukocyte- and platelet-rich fibrin-enriched nanograft (L-PRF) during surgery, which improves skin thickness and promotes tissue regeneration.
- In Type II patients, it uses techniques that help control swelling and improve nasal contour definition.
Positive and Promising Results
In a study conducted between 2022 and 2023, patients treated with this method showed significant improvements:
- An average increase in skin thickness of 0.9 mm in thin-skinned patients after one year.
- No cases of nasal dorsal irregularities requiring surgical revision.
- All patients were satisfied with the results, with no relevant complications during the recovery process.
A Step Forward in Plastic Surgery
This method not only improves aesthetic results, but also reduces the risk of postoperative complications. By combining traditional surgical techniques with modern regenerative medicine tools, such as nanograft and L-PRF, Dr. Garcia Ceballos’ approach demonstrates that it is possible to customize procedures to meet the specific needs of each patient.
