Prosthesis Replacement with Capsulectomy

One of the most complex surgeries

Prosthesis replacement is an operation performed on people who have previously undergone breast augmentation surgery.

Although it may seem a simple operation, it is possibly one of the most complex surgeries that the breast undergoes.

Las prótesis mamarias no duran para siempre. A woman undergoing breast augmentation surgery for the first time should be aware that she may need additional breast surgeries during her lifetime.

After several years, 10, 15 (and even 20 years in some cases) breast prostheses may need to be replaced by new ones.

Reasons for prosthesis replacement

  2. Capsular contracture: involves the transformation of the breast into hard, round and in some cases painful.
  3. Prosthetic rupture: if the rupture of the prosthesis is confirmed by ultrasound or magnetic resonance imaging, a replacement or at least removal of the implants is recommended as soon as possible, although it is not an emergency.
  4. Displacement: silicone-coated prostheses can be displaced downward or sideways.
  5. Aging of the breast: this can produce a “rare” double bubble breast shape due to slippage of the breast over the breast implant.

Prosthesis Replacement

In general, polyurethane-coated prostheses tend to have fewer complications than silicone-coated prostheses. Dr García Ceballos has extensive experience in the implantation of polyurethane prostheses.

The replacement of an implant with an anatomical implant with silicone covering can lead to a higher risk of implant rotation, in these cases the use of anatomical polyurethane implants is recommended, which avoid the risk of rotation.

Dr. Garcia Ceballos is an expert in the implantation of polyurethane breast implants and currently, 95% of his patients choose this material. The choice of the type of prosthesis or material can be made by you according to the information received from your surgeon.

In some cases where the patient’s breast is excessively small, it may be advisable to increase the protection of the new prosthesis by using a biological matrix to reduce the risk of visible wrinkles.

The Polytech brand guarantees lifetime replacement in case of rupture of the implant shell for silicone gel-filled implants. For implants covered with polyurethane foam, in addition to lifetime replacement in case of rupture of the implant shell, an additional warranty of a free Polytech brand replacement breast implant is offered for up to 10 years after implant placement in case of grade III and IV capsular fibrosis or implant rotation/dislocation.

The doctor’s recommendation is to replace silicone covered prostheses with polyurethane ones to avoid complications such as capsule, displacement and rotation as much as possible.

Currently, Dr. Garcia Ceballos recommends the removal (capsulectomy) and analysis of the capsule in order to reduce the risk of capsule in the new implant and to analyze the risk of anaplastic giant cell lymphoma.

After the operation, the patient must wear a postoperative bra and an elastic band for one month in the case of silicone-coated prostheses, and only the postoperative bra for polyurethane-coated prostheses. It usually requires an overnight hospital stay.

Usually these operations tend to produce more bleeding than the initial breast augmentations; this means that it may be necessary to keep the drains in the breasts for a few days until the fluid production is small enough for the body to reabsorb it.

The result can be seen one month after the intervention, but it takes at least one year to appreciate the final result.

The final result depends on the body and breast characteristics of each patient, as well as the wishes of each patient.

FAQ
Frequently Asked Questions

The postoperative replacement of the prosthesis is not very uncomfortable and does not usually hurt if compared to a breast augmentation in its first intervention, drains are recommended to control the level of bleeding and facilitate a faster recovery.

The duration of breast prostheses does not have a fixed period. It is common to confuse the performance of a revision operation after a breast augmentation with the duration of the prostheses, which is the time that the prostheses remain intact.

Reoperation after breast augmentation may be due to other complications that are not related to the integrity of the prostheses such as capsular contracture, displacement of the prostheses, sagging of the breasts due to the passage of time or the patient’s desire to change the shape and/or volume of her breasts.

The risks of prosthesis replacement or removal depend on the procedures performed in conjunction with explantation.

Before asking this question, we should consider what is the best option after the removal of breast prostheses. In this sense, it is important to attend to the needs of each patient. There is an option that we could define as “healthy” which would involve removing the prostheses and reconstructing the breast that remains without prostheses, and another option that would be “aesthetic” although “less healthy”, which would involve replacing them with new prostheses. This requires explanations and discussion with each patient.

As mentioned above, there is an option that we could define as “healthy” which would involve removing the prostheses and reconstructing the breast that remains without prostheses, and another option that would be “aesthetic” although “less healthy”, which would involve replacing them with new prostheses. Either option is always possible according to the needs of each patient.

Encapsulation is not “per se” an indication for implant replacement unless it causes breast deformity or pain, in which case it is up to the patient to decide whether or not she is able to tolerate such deformity and/or pain.

The capsule is a fibrous tissue that the body generates around the breast prosthesis in response to the stimulus that they represent. The capsule has the capacity to contract to a greater or lesser degree and thus deforms the prosthesis, causing the breast to become hard and irregular. Definitive treatment is performed by surgical removal.
