Facial lipofilling is a surgical procedure in which fat is extracted from a part of the body and implanted in the face in order to provide volume and improve the appearance.

With the passage of time, facial tissues lose volume, lose fat, and lose bone volume.

There are different ways to recover the lost volume, one of them is by using one’s own fat.

It has been widely accepted for several years that the use of the patient’s fat as a facial filler is a good tool to rejuvenate the facial appearance by providing volume in a lasting way.

The procedure for the use of fat as a facial graft in what we call facial lipofilling.

Candidates

People who can benefit from this type of treatment must have enough fat in some part of their body to be able to extract it, process it, and inject it as a filler (fat graft).

It consists of an extraction through liposuction and subsequent processing of fat for grafting, which is called lipotransfer.

The volume to be extracted for a complete facial filler can be around 50 cm3.

The procedure for the extraction of fat, which will later be used as facial lipofilling, is performed in a different way than liposuction to reduce the volume of a specific area of the body. The difference lies in the fact that the cannulas used for the extraction of this fat graft are thinner.

Fat processing: once the fat has been extracted, it is processed. Nowadays we can elaborate different fat preparations depending on the filling needs we need to cover. In this way, it is possible to produce a grease of greater or smaller size, even becoming liquid(nanofat).

We also know that if we add growth factors, particularly LPRF, to the grafted fat, the viability of this fat increases.1,2

Anesthesia: this amount could be performed by a procedure under local anesthesia for fillings that are not too intense. It is possible to perform them under general anesthesia when the amount of fat to be extracted and grafted is sufficiently large and an improvement of the treatment conditions is estimated.

It is normal for some bruising to appear in the liposuction-grafted areas after the operation.

Usually the postoperative period in the area of the fat graft is not uncomfortable, although there may be discomfort and bruising in the liposuctioned area.

After surgery, a compression garment or bandage is applied to the area from which the fat has been removed. at one week and 14 days after the intervention we performed a new application of growth factors in the grafted fat to improve the viability of the fat. This will cause your facial appearance to appear puffy during these two weeks to a greater or lesser degree.

During the postoperative period, we offer the possibility of treating bruising with a laser one week and one month after treatment.

It is precisely the permanence of the injected fat that is one of the disadvantages we must be aware of before undergoing this procedure, since part of the fat may disappear during the postoperative period.

In any case, the regenerative capacities of both the fat and the growth factors incorporated mean that we benefit from both the volume provided to the grafted area and the quality of the skin.

With the passage of time this edema settles and the definitive result can be seen 2 months after the intervention.

If the patient desires more facial volume after 3 months and there are sufficient fat deposits in the patient’s body, the procedure can be performed again to increase it.
